Our Innovation Hub model allows us to be integrated within our ICSs to help understand the unmet needs that can be solved through working together to adopt innovation.
The hubs provide dedicated innovation support and a conduit through which innovators can engage with system colleagues in a managed and efficient manner.

With the support of our AHSN partner, we can demonstrate the implementation of evidence-based interventions that we estimate have prevented 1,000 readmissions to hospital; one case of cerebral palsy; 125 strokes through the introduction of mobile electrocardiography and associated services; 10 heart attacks and another 14 strokes through roll out of the West Yorkshire and Harrogate Healthy Hearts programme.
We have been wanting to improve communication with our community pharmacists for some time. Funding was an issue, so we were really pleased when the Yorkshire & Humber AHSN was able to support us in delivering the Transfer of Care Around Medicines programme.

Safety huddles have had a massive impact for both our staff and patients in Leeds Teaching Hospitals. Everyone has a voice and this creates great team work. Huddles really do improve the safety of our patients.
